| About Community Alternatives Program: | |
| | CAP is an alternative to facility placement for adults and children. Recipients must be Medicaid eligible, meet criteria set forth by the Division of Medical Assistance, need facility level of care and have adequate informal support. Services include case management, medical screening, assistance with personal care and home management needs, securing medical supplies and home mobility aids, referrals to other community organizations. |
